The main US guideline for assessing and reducing cardiovascular risk in adults without established disease. It consolidates prior statements into one document covering risk estimation, lifestyle, and when to consider preventive medication, alongside team-based care, shared decision-making and social determinants of health.
2021 ESC Guidelines on cardiovascular disease prevention in clinical practice

The ESC's current prevention guideline, covering risk factors, risk classification and stepwise prevention of atherosclerotic cardiovascular disease at both individual and population level. It is paired with the SCORE2 and SCORE2-OP risk calculators and the HeartScore tool.

US vs EU key differences
- Risk tools differ: the 2019 ACC/AHA guideline estimates 10-year ASCVD risk with the Pooled Cohort Equations in adults 40-75, while the 2021 ESC guideline uses SCORE2 and SCORE2-OP, which capture fatal plus non-fatal events and are recalibrated to four European risk regions. source
- The ESC applies age-banded treatment thresholds rather than one cut-point, treating apparently healthy people as very high risk at SCORE2 above roughly 7.5% under age 50, above 10% at ages 50-69, and SCORE2-OP above 15% beyond 70. source
- The European guideline introduces an explicit stepwise treatment-intensification model, moving from baseline goals to intensified lipid, blood pressure and glucose targets, whereas the US guideline refines borderline and intermediate-risk patients using risk-enhancing factors and coronary artery calcium scoring. source
- On aspirin the US guideline is notably restrictive, stating it should rarely be used for routine primary prevention because of the lack of net benefit. source
- Version currency differs: the American primary-prevention document dates from 2019, while the European prevention guideline was issued in 2021 and folded in the newer SCORE2 models. source

The 2021 ESC prevention guideline replaced the SCORE fatal-event chart used since 2016 with SCORE2 and SCORE2-OP, which estimate combined fatal and non-fatal cardiovascular risk, are calibrated to four European risk regions and for the first time cover adults over 69. It also introduced age-specific risk thresholds and a stepwise, shared-decision approach to intensifying lipid, blood pressure and glucose treatment.
