An AAO-HNSF guideline on patient selection and surgical indications for tympanostomy tubes in children aged 6 months to 12 years with otitis media of any type, in any care setting. It addresses when watchful waiting is appropriate for middle-ear effusion, when hearing testing is warranted, the role of adenoidectomy, and postoperative care and follow-up. Published in Otolaryngology-Head and Neck Surgery in 2022.

    The 2022 update reassesses the 2013 guideline against 18 new systematic reviews and 27 randomised trials, keeps the bar against tube insertion for a single effusion episode lasting under three months, requires a hearing evaluation once effusion persists three months or before surgery, and adds a new strong recommendation that the surgeon or a designee re-examine the child's ears within three months of insertion and set expectations for periodic follow-up.
