Living EuroGuiDerm Guideline for the systemic treatment of psoriasis vulgaris

The EuroGuiDerm living guideline on systemic treatment of moderate-to-severe plaque psoriasis, hosted by the European Dermatology Forum. It covers biologic and non-biologic systemic drugs with treatment algorithms and monitoring advice, plus guidance for comorbid situations such as psoriatic arthritis, inflammatory bowel disease and prior malignancy.
US vs EU key differences
- The European guideline embeds explicit numeric treatment goals - PASI 75 as the minimum response and PASI 90, absolute PASI under 2, or DLQI of 1 or less as the desired goal - rather than organising guidance drug by drug as the AAD-NPF series does. source
- The European document also fixes when to judge those targets: after 10-12 weeks of induction for fast-onset drugs, 16-24 weeks for slower ones, then every 8-12 weeks during maintenance. source
- Update model and drug coverage differ: the European guideline is a living document (comprehensive update 2023, partial update 2025) that already covers deucravacitinib and other newer systemics, whereas the AAD-NPF series was written in 2019-2021 and predates those approvals. source
- Structure differs: the AAD-NPF guidance is split across six modality-specific papers (biologics, systemic non-biologics, topicals, phototherapy, paediatrics, comorbidities), while the European guideline is one integrated systemic-therapy document with a decision grid for comorbid situations such as psoriatic arthritis, inflammatory bowel disease, cancer history and pregnancy. source
What changed recently
- EUSeptember 2023 version → February 2025 partial update2025-02-01
The February 2025 partial update to the living EuroGuiDerm psoriasis guideline changed recommendations in the viral hepatitis chapter and rewrote the pregnancy and wish-for-child chapter around new data, adding pregnancy content to the main recommendations and decision grid. It also revised the laboratory monitoring advice for deucravacitinib.

The joint AAD-NPF series was the first rewrite of the 2008-2011 AAD psoriasis guidelines in over a decade, produced with the National Psoriasis Foundation. It devoted a dedicated section to biologic agents with updated timelines for judging treatment response, and broadened the scope to comorbidity screening, paediatric psoriasis, phototherapy, non-biologic systemics and topical therapy.
